When you insert the second voice, at times, the dot disappears.

On the other hand the behavior when there are a unison with notes of different duration, should be not share heads. One should share notes whose duration is the same. See examples.

Only the 2nd is correct.

There are two bugs here. The first one is about the dot disappearing and it's a bug for sure.
The second is arguable. According to Gould p59, 1 and 3 would be acceptable. Regarding 4/ I'm not sure how you created it but it's probably not the default behavior of MuseScore.
