How to put save icon back in the toolbar?

Same way you presumably removed it - View / Toolbars / Customize Toolbars. Unless you actually mean you close the entire File Operations toolbar. In which case, just re-enable it, also in View / Toolbars.
Note the save icon doesn't do anything different from File / Save or Ctrl+S.
