3 Note Tie Fail

So once I reach measure 25 in this score, tying 3 notes (a chord) together causes the middle note to do a very bizarre thing where it creates a large solid shape in the wrong direction and sometimes changes the spacing, both vertically and horizontally, of the staves. I have tried this on two separate computers, and it happens at the same point. At first, I thought it was because I was cutting and pasting, but it also happened when I try to reenter the notes manually. Individually, I can tie the top and bottom notes together, but it happens when I try the middle note as well. I can also say that in the previous measure, I used layer 2, and this is the treble staff of a piano part. I have used a midi keyboard and regular keyboard entry. Help. :)
