Chord Symbols and Mystery added notes

Hello,

While doing a transcription of a Lawrence Welk piece for a book I am writing, I found an error with the playback feature. On the second page, I have the chords C-6, G/D, and D7. There are extra notes being added on top of these chords in the playback. This seems to happen when I play back the audio more and more. Upon closing and restarting the software it clears it up but in time returns and keeps adding notes to other chords. Here is what is added after a 10 playbacks.

C-6 has a phantom E natural added
G/D has a phantom C and A above added
D7 has a phantom C# added

The mystery notes all show up even when I delete the chords and re-insert them. They still appear when I uncheck the play feature for the chord symbol in properties. I do not want to start from scratch as this has taken a good amount of time to transcribe. What could be the issue and how do I resolve it? Thanks
