Audiveris plug-in not working.

The above link is the plug-in I am trying to use.

The above link is a previous thread about the plug-in. It covered a problem that I am experiencing, but the fix is not working for me. The problem I have is whenever I upload a pdf to be converted, it says "Line 89 'form' [undefined]' is not an object".

These are the 2 files for the plugin. Line 89 is referring the the audiveris.js file.

Is there anything I can do? i have the UI and the JS file in the plugins folder.
