Repeats and voltas – I'm missing something...

I'm working on a score (really my play notes), trying to boil it down to the bare essence, and I can't quite get it to play the way I see it...

OK, line 2 (and line 15) are the “hook” and below those lines are the endings of the “hook”. I can get it to play repeats 1, 2, 3, in the proper order but it completely skips repeat 4.

The “hook” at line 15 is the same idea except that I have separated repeat 4 into line 28, but it still skips repeat 4.

What am I missing, what have I overlooked?
