Playback ignoring multiple endings

I have had this happen several times; I will use 3 or 4 ending with repeats and the playback works just fine. Then, out of nowhere, the various endings are ignored, and upon repeat, it keeps going back to the first ending. No changes were made to the piece. This has happened at least 3 times and affects every piece that has repeats. All of a sudden, several weeks or months later, the problem spontaneously resolves. I've attached a sample piece which would not properly repeat today.
