Problems with Semi-Quaver Rest

So, I'm just starting to learn how to do this but I have struggled to get the rests in the correct place.
Take a look at the attachment carefully and you will see the rest hiding behind the treble clef on the first line and then off the stave on my second attempt below.
I'm not sure that I can repeat all the steps but in the first case I placed the cursor before the first quaver, type 3 for a semi-quaver and then hit the rest icon and this is what I got!
I am running version 1.2 on mac os 10.7.5.
BTW the application refuses to place a beam between the single semi-quaver and the following quaver, maybe this is related?

Seeing the actual score rather than just a picture of it would help. Do note that MuseScore has no concept of "inserting" rests or notes - everything is in "replace" mode. So following the steps you descriebd should have resulted in the rest stealing a quarter of beat from the crotchet.

As for beaming, see the section Beam in the Handbook. You can break or join beams anywhere you like.

I suspect the score is corrupted. Even v2 (development) loads it badly. If you export as XML and reload the XML file things seem corrected.
