Tied note sounds twice in the playback

In the attached, the A sounds twice and the C's do also. These are clearly tied notes and should not sound a second time. The tie is not a slur. How I input the tie does not seem to matter. s or double clicking the line - or trying to fake a dotted quarter note and change the eighth into a sixteenth. It always sounds both notes on playback.

What am I failing to do?
