Courtesy accidental - seems inconsistent

I have the Courtesy Accidental plugin enabled for my current score, but it did it for one occurrence, but not the other. I read the manual and it doesn't actually say how to invoke a plugin. I can see "addCourtesyAccidentals:
this plugin will add courtesy accidentals to the selection or to the whole score, if nothing is selected. It uses the "next measure, same octave" rules." (MU3). * Is there an MU4 version of the manual for this? I couldn't find it.
But does this mean I need to keep double-clicking on the plugin icon, or is the plugin constantly scanning the note input?

In the test example below, we have no CA for cases A, B, C, and a CA for case D. This is whether the plugin is enabled or not. Including doing a 'Reload', which I don't actually understand. I would prefer documentation to be a bit more precise and unambiguous for people like me who need their hand held. :) I don't know if the above is a bug or my ignorance of how the plugins work. Perhaps I've got the CA definition wrong. It's not a big issue, but at least I would like to have it consistent.

Well after a lot of experimenting I am starting to understand how this CA (= Courtesy Accidental) works in MU4.
1. After I did Configure, it started to work.
2. I had to select both the bar and the preceding bar. —This makes sense, since the plugin does not have a global view.
3. Assigning shortcuts circumvented messing with those big icons — much better.
4. Although in the Configure I have ticked the parentheses box, it is ignoring this. I have not found any workaround, since "(#)", for example, is not supplied as an accidental.
5. With one of my scores , it produced a proper CA with parentheses. I tried to drag this to the Custom palette but no go.
6. At one point, it stopped working. I had to quit MU4 and reopen.
7. I like the parentheses version, so I am still WITHOUT a proper CA feature.

What am I missing??

Is CA configured on a score by score basis?

Discovery: NOW I am getting the parentheses, if I do Configure and specify 'Add accidentals'.

This is dysfunctional. 'Configure' normally means "Specify the parameters of operation." There should be NO CHANGE to the score (in this case). This needs redesigning.

Look at the trouble it has caused me because it is so counter-intuitive. The only way to get it to work is to go through 'Configure'. I guess that is workable since doing CA is a one-off sort of operation. Now that I know ...
