The underlined note mentioned in image 1 is heard as a (really quick) grace note for the next note.

The problem is that the Ab 4 in question is already being played in the bass staff. Fonts don't seem to be able to do that. To show what I mean, make that chord an 8th note instead of a 1/4 note. The treble measure will play properly.
You could also change the Ab in the bass staff to the 2nd voice and flip it (X) to match the picture. This will play both the notes while looking the same as the picture. (You may need to delete/hide extra rests if you do this (V))
