Can't create custom key signature

Mac OS X 10.5.8

I went to create a key signature to create a key signature with a F# and a B-flat. I am able to drag a sharp and flat to the staff. When I click the "Add" button, a new blank staff is added to the left-hand window but no new custom key signature.
