When entering first and second voice within a bar the horizontal spacing between the signs is to small, such that they overlap.

You can move the accidental by double clicking it and using the arrow keys on your keyboard

tweaking it by hand might work when it happens just once, but imho it should be fully automatic. Tweak-by hand may also result in uneven distances
