Help with Volta - Endings not playing back correctly

I'm having trouble getting the 1st and 2nd ending to play correctly. When the file plays back, it plays the first ending once, and the 2nd time around it skips over it and jumps to the 3rd ending. I acknowledge that the music looks correct and a human would play it correctly but I really would like to have the file play it back correctly.

I did update the play count to '3'. Other than that, I'm not sure where my mistake lies. Thank you for your help!

The end goal here is to produce a MIDI file for my piano player so she can practice the music along with the MIDI file. I've added the DS al Coda, Coda, etc. I just can't get the file to play back correctly.

I inserted 2 measures and deleted the two (corrupted?)

If you view your first attachment in Continuous view and drag the first volta slightly upwards, you will see a bogus second volta beneath it. This was confusing for playback because its repeat list is set to 1.

P.S. Your second attachment shows the same.
