Any idea what this is?
I am annotating some ukulele music so i can play it back in the car and there is this strange thing with two notes that seem to be the same notes in the same place. Needless to say I am struggling to put it into musescore, even using the 4 voices. I presume it is there for a reason and if i knew what it was then that would be something. Any ideas?

Nothing of particular. It's simply due to the re-entrant tuning of the ukulele: https://en.wikipedia.org/wiki/Ukulele#Tuning
On your image, you are playing a Am chord. So, by pressing the second fret (4th string), you get a pitch of A4. And, as by chance, the first open string is also a A4. So, these two A4 are shown on the score.
If you want avoid that: simply press "V" key eg on the second A4.
Enter the same note twice and for one mirror its head using the Inspector or the Shift+X shortcut
