Playback plays both D4 and D5 instead of just D4.
If I open up a new project and write a D4 in bass clef. I hear a D4 + D5 during playback.
If I write a D4 in treble clef, I only hear a D4. I have installed and reinstalled the software. Does anyone else have this issue?

Plays OK for me. Tell us more about your setup. What font?

Is there a way to change the playback instrument?
Maybe I'm just hearing the overtones from the Musescore piano playback - the D4 overtone seems especially strong for some reason though. I recently upgraded my audio hardware setup - maybe that's part of it.

In the Mixer (F10) you can select a new instrument sound from the Sound dropdown list.
That's not a bass clef staff, but a treble clef one with a change to bass clef.
Not that it'd make any difference in how it sounds, which is correct to me.
Reinstall would have kept all your settings, maybe some non-default soundfont? Reverting to factory settigs would get brid of that
