Institutional Vetting Process

Good afternoon!

My community college employer must vet any proposed software/programs before allowing faculty to download them to their work devices voluntarily. The college gave me an application form to begin the vetting process. I need the following information to complete the form:

Vendor company legal name
Vendor Cybersecurity name, email address, and phone number
Product type (pertinent options include: installed software or software as a service)
