incorrect notes are played on playback

In a saxophone sextet I arranged, in the third and fourth to last bars, the second alto part seems to play back the low D# as both the D# and for some reason the D natural below it as well. In some cases this issue could likely go under cover, but in this cases it changes the chord entirely so i am curious how to resolve the issue. deleting and re-entering the faulty note makes no change.
