In the enclosed score, Cmd-B appends 4/4 measures, not 3/8 as the time signature calls for.

It doesn't for me, although the last four measures report as being corrupt and actually in 4/4, so that's probably related. Which build are you using? Please update to today's version of the beta (Help / Check for Updates) and see if you can still reproduce. Might need to delete those corrupt measures though. I take it those were created by appending measures and that's why you saw them as 4/4. I see a bug fixed before the first beta but after the first alpha that could explain this, if you were ever using one of those builds: #262214: Appending measures uses 4/4 time signature

This was all today, in 19499. I just updated to the brand new one FWIW.
OK, well if you can provide precise steps to reproduce the problem that led to the corruption, that would help.
