How did this file corruption occur?

What does the message exactly mean? :

It means what it says. That measure has in 1 8th and 2 quarter notes and a half rest in voice 2, that is too much for a 4/4 measure
It probably should look like this
As for "how" did it occur, we're hoping you can tell us. If we know what causes a corruption, high priority is given to fixing that bug so it doesn't happen in the next version any more.

Can it be that your original source is from a pdf conversion, because in this case such corruptions occur very often since optical music recognition is seldom perfect.

I don't think so, there's no evidence for this, not in the score properties, nor in the score,

It looked like that because I haven't even started arranging yet.
So no, all of the notes are manually placed.
I used only 2 voices (blue and green notes), different tempos and only 2/4 & 4/4 time signatures.

I have no idea either.
I was just transcribing a song's drum onto a piano sheet for reference. I wasn't even in the phase of arranging yet.
I used 2 different voices (the blue and green notes). That's all there is to it.
Other than that, you should notice that I used a lot of tempo and changed time signatures before. Not sure if that directly caused this.
