Before saving the attached file I took the screenshot attached.
If you open the file, you will see a double flat instead of the flat.

Its a layout problem. MScore finds out too late that two notes share the same note head and only one accidental should be used. When the layout routine is called again (after any editing) all is well.
This is fixed in rev. 3214.
