Full Screen Exit Button Still Disappearing
The exit button on Windows is still disappearing if I double click on the window header to make it full screen. Clicking the full screen button is working as expected.

I leave View/Full Screen unchecked. You can then drag the window to the size you want. Double click on the header should then work correctly.
This is a known glitch when using multiple monitors and starting it on a display with no taskbar if there is a taskbar on the main display.
