Can't get Voice icons on toolbar

I can't see Voice 1 and 2 on the toolbar, even though the eyes are open. I've looked in the online handbook and I can see a Voice icon visible on the toolbar in the example under the heading 'Adding Items form the palettes' but I can't get one on mine. What am I not doing?

That's what I was doing but the icons didn't appear, whether the 'eyes' are open or shut

OK I've managed to get one on Voice on there using the Up and Down arrow but when I try to add another one, it replaces the first one, I can't get more than one there at the same time

Could it be a space problem?
Close the nonessential eye icons or unhook the panel by dragging it by the six points in the upper left corner.

I've managed to get voices 1 and 2 there now by opening voice 3's eye but leaving it on the drop down menu and not trying to add it to the toolbar!

??? I'm not sure what you mean by this. When you open the Customize Toolbar list, you add an item to the toolbar by "opening the eye". I don't understand what you mean by "leaving it on the drop down": when you "open the eye" of a given toolbar tool, it stays "on the drop down".

If not already done update to 4.5.1, then try Restoring to factory settings.

I've already updated to 4.5.1 as the previous version I had was crashing on opening but I will try as you suggest
