how would this transition work?
I feel like the slurs won't fit but maybe some assistance on how it would work would help.
I can probably make the transition but I just want to understand how the transition works.

Looks like ties to me, at least partly (at least 3 of the 6).

since the previous chord has 6 notes and the other one has 5, what note will one of the 6 notes connect to?

No matter how you slice it, it's incorrect. For one thing, the lines should have been continued onto the next system. For another, some of these at least are clearly slurs and not ties, since there is no corresponding note in the second chord to connect to, and yet they all notated the same length, which is wrong (the slurs should go "outside" the ties). And slurs should not be connecting individual notes - they should connect the chord as a whole (that's a rule that is broken sometimes when necessary, but it doesn't seem to be here).
My best guess is the two Eb's on the bottom staff are meant to be tied, whereas the chords on the top staff (including the cross-staff Ab on the first chord) is meant to connected to the next via a single slur.
