Hopefully I can describe what seems to be wrong (please correct if I'm wrong):

It seems that one edge of the set is not the correct height, and/or there isn't enough space between the grace notes. As a result, the oblique stroke in the first set isn't displaying correctly either.

Here is LilyPond (the oblique stroke isn't there):
