No instrument names and measure numbers in continuous view
OS: Windows 10 (10.0), Arch.: x86_64, MuseScore version (32-bit): 3.0.0, revision: d1e759d
Switch any score to continuous view and scroll right to hide first measures.
Result: there are no "blue" instrument nor measure numbers.

relates to #272135: [EPIC] Continuous view issues
For some reason (probably because they are not drawn), the measures are not inserted in the bsptree anymore.
Also stafflines are not drawn the same anymore and layout compute the width.
