It's a little better after I reloaded the score, but the accidental attached to the trill line is still intersecting with the phrase/slur line from the system above. It's at measure 181, high Eb, Violino I.
To recreate fully, create a new score, arrange it so a down oriented slur is on the bottom of one system, and a trill line is at the top of the next and they're on the same page. Probably works better if there are ledger lines involved.
It seems that reloading scores also resets layout - I can't give you an example of how egregious it was last time. Next time, I'll screenshot it before reloading.
High five to whoever fixed it so that trills now actually play accidentals. My ears thank you.

Yes, I would say the likely conclusion here is that the accidental attached to a trill is not added to the "shape" for the trill.
