Create a new sheet and put in time measure 12 / 4. Then keep filling in crochets in the bar. Only 23 are filled in instead of the expected 24.

Of cource a 12/4 measure can't hold more than 12 crotches AKA quarter notes, I guess you mean quavers AKA 8th notes?

Whatever, that measure has an actual duration of 23/8 (see also that little "-" sign the its top right), so this should be by design
