Inputted notes that sound different than the note that was input

When inputting certain notes, they don't always play the same tone that one inputs. On the attached file, look at the piano treble clef and violin notes in measure 3, 2nd beat; measure 11, 2nd beat. There are a few more places where inputted notes play a different tone than where the note was placed on the staff. I'm not sure how to change this. Thanks for any help you can give.

Status won't fix by design

The "Db" in measure 3 beat 2 was entered as an Ab and moved in the inspector. The y offset in the chord section is set to -1.40sp. If you select all of the notes and press ctrl+r they will return to where they are actually entered.

Most likely, you use the mouse for note input and are used to dragging a note up to change its pitch. If you accidentally double click the note and drag it, it will move the note without changing its pitch and the y offset will be adjusted.
