Full measure rests and others?

Sometimes in a score there are full measure rests like this:

I have a few questions regarding this code:

1) Is this something that the user chooses to do or is Musescore doing this automatically?
2) Is there always a duration tag after the durationType tag?
3) Are full measure Chord tags ever used or is this only for rests?
