8va (ottavo) lines won't draw over some measures

I have a problem in a score (being set from a manuscript in Library of Congress) where the ottavo lines won't draw above certain measures. I've had to manually draw a stave line in, as shown in measure 3 of the attached extract.
The ottavo style has "Show Line" checked.
Repro
1. Select measure
2. Click "8va alta" in Lines palette
Result: 8 shows, but no line. The pink markers show at start and end of measure.
