tie randomly turns into slur

I'm having an issue where ties inexplicably change position from the intended note to another note several beats away after reverting to a saved file and sometimes when entering new notes.
I scanned through the previously listed bugs relating to ties to see if this had been addressed but didn't come up with anything.
I'm running version 0.9.5 on Windows 2000.
Any help would be appreciated.
