Working in Bass Clef on French Horn and at one point, no matter if i would bring the previous note back down the octave, the next note I would input would automatically be an octave above the last.
Edit: I just figured out it happens when transferring from one measure to the next, if that helps.

I've seen it, reported it, but can't make it happen on demand. If you can it would help.

Since that issue was originally about something else, let's keep that focused on the original issue (handling of accidentals sometimes throwing off the "closest octave" calculation by a step), and use this issue to track this new, more serious but also more sporadic problem.

I changed the bass clef line to treble clef with no issues. Then I changed it back to bass clef and suddenly, instead of getting bass clef pitch like I expected, I'm still getting treble clef pitch and I have to press ctrl-down at least once to get it in the right octave. I don't know yet if this happens every time that you change back to a clef you originally were in but I do know that it happens at least for bass clef -> treble clef -> bass clef and as soon as I get to the next measure, it goes back to being at treble clef pitch. This is very frustrating when I am transcribing a piece like Spring from The Four Seasons for piano and flute.

Wait, it just resolved itself for me. But that doesn't mean it won't happen again if I change clefs.
