The example shows two system texts. The first is on the note of beat 3, and can be read by NVDA. The second is at the half of the half note, on beat 3 too, but unreadable by NVDA, even by using alt+arrow.

Good observation! It's normally pretty difficult to create this type of situation for ordinary text but can be done through careful editing. And perhaps through MusicXML import? It is a common situation (and easier to enter directly) for chord symbols, though. We would need the next-element command to check for annotations on the current staff even if there are no notes there. So, cases where there is a segment but no note.

Doing this in Musescore first putting two quarter notes, entering text on the second, then change the first note to half. Both text and chord symbol not on the beginning tick of the note are not readable by shortcut.
